new lawn going bald and muddy

hi all
I recently laid a new lawn 4 months ago but its all gone wrong. first the grass started coming out an now its just mud and messy i really need some help and tips on wat 2 do so please be my saviour and make my day thanks

Can you tell us what your watering practices were when it was planted? What do you mean by the grass started coming out--it was growing or it was easily pulled out? Is the grass that remains brown and dormant?

Need more info, please!
-- Where are you located -- Dublin, OH or Dublin, Ireland?
-- What kind of grass are we talking about? Fescue, ryegrass, bluegrass?
-- Was it sod or seed? Did you do it yourself or did someone do it for you?
--- What have the growing conditions been like since you had the lawn created? If you are in Ohio, that means you put the lawn down very late in the season, maybe too late to hope for it to survive a hard winter.
--- How did you prep the soil before you sowed the seed (or laid the sod)? Bed prep is extremely important.
